You are hereby given Notice that a Complaint seeking a Name Change of Minor from you was filed in the Circuit Court for Macon County, Tennessee on November 4, 2016.  You are summoned and required to serve upon Tyler Whitaker, Plaintiff’s attorney, whose address is 601 West Main Street, Lebanon, TN  37087 within thirty (30) days after publication of this notice by February 27, 2017.  Also, a copy of the answer must be filed with the Circuit Court Clerk at his office at 904 Hwy 52 By-Pass East in Lafayette, Tennessee either before or within a reasonable time after service.  If you fail to do so, judgment by default can be taken against you for the relief demanded in the Complaint.
